Responsibilities:
- Interview patients to identify their medical and social service needs.
- Educate patients about their condition, medication, and give specific instructions.
- Develop a care plan to address their personal health care needs.
- Respond to patient’s questions and expressions of concern.
- Comply with hospital and legal requirements by fostering nursing practices that adhere to the hospitals and nursing division’s philosophy, goals, and standards of care; requiring adherence to nurse practice act and other governing regulations.
- Work as part of a team with other members of the interdisciplinary team.
- Locate and contact providers of medical care.
- Facilitate the coordination of social services for patients.
- Perform other duties as assigned
Qualifications:
- Proficient in Medical Assistance, Case Management, or Social Services and Patient Coordination (min. 3 years of experience)
- Familiarity with Medical Terminology and HIPAA Regulations
- Understanding of Healthcare Privacy Laws and Compliance Standards
- Flexibility in Adapting to Processes and Technological Changes
- Exceptional Attention to Detail and Organizational Proficiency
- Proficient in Communication and Problem-Solving Skills
- Demonstrated Experience in Managing Complex Cases, including Chronic Physical Health Conditions, Mental Illness, Substance Use Disorders, Homelessness, Justice Involvement, and High Utilization of Multiple Health Systems.
