• Interview patients to identify their medical and social service needs.
• Educate patients about their condition, medication, and give specific instructions.
• Develop a care plan to address their personal health care needs.
• Respond to patient’s questions and expressions of concern.
• Comply with hospital and legal requirements by fostering nursing practices that adhere to the hospitals and nursing division’s philosophy, goals, and standards of care; requiring adherence to nurse practice act and other governing regulations.
• Work as part of a team with other members of the interdisciplinary team.
• Locate and contact providers of medical care.
• Facilitate the coordination of social services for patients.
• Perform other duties as assigned

• Proficient in Medical Assistance, Case Management, or Social Services and Patient Coordination (min. 3 years of experience)
• Familiarity with Medical Terminology and HIPAA Regulations
• Understanding of Healthcare Privacy Laws and Compliance Standards
• Flexibility in Adapting to Processes and Technological Changes
• Exceptional Attention to Detail and Organizational Proficiency
• Proficient in Communication and Problem-Solving Skills
• Demonstrated Experience in Managing Complex Cases, including Chronic Physical Health Conditions, Mental Illness, Substance Use Disorders, Homelessness, Justice Involvement, and High Utilization of Multiple Health Systems.
