A powerful new Medi-Cal benefit called <\/span><a href=\"https:\/\/www.dhcs.ca.gov\/formsandpubs\/publications\/oc\/Pages\/010526-Stakeholder-News.aspx#:~:text=Top%20News,person%2Dcentered%20behavioral%20health%20system.\"><span style=\"font-weight: 400;\">Transitional Rent <\/span><\/a><span style=\"font-weight: 400;\">is now mandatory for all Medi-Cal managed care plans (MCPs) \u2014 and it could mean the difference between a stable home and a cycle of crisis. At Pacific Health Group Community Support Services, we&#8217;re here to help members understand this benefit, access the services they need, and build a bridge to lasting housing stability.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">What is Transitional Rent, and why does it matter?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Transitional Rent is the newest addition to California&#8217;s CalAIM Community Supports program \u2014 and it&#8217;s the first Community Support to become mandatory for all Medi-Cal managed care plans statewide, effective January 1, 2026. This benefit provides up to six months of rental assistance for eligible Medi-Cal members who are experiencing or at risk of homelessness.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Unlike emergency department visits or nursing home stays that address health crises after they happen, Transitional Rent is a preventive, whole-person care approach. It recognizes what public health researchers have known for years: housing is one of the most powerful social determinants of health. When people lack stable housing, their physical health, mental health, and overall well-being suffer. Chronic conditions go unmanaged, medication adherence drops, and the risk of costly hospitalizations rises.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Transitional Rent directly targets this gap \u2014 keeping vulnerable patients housed so they can focus on their health care, not their next place to sleep.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">Who qualifies for Transitional Rent?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Eligibility for Transitional Rent is based on three key criteria. A member must:<\/span><\/p>\n<ol>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Meet clinical risk factors<\/b><span style=\"font-weight: 400;\"> \u2014 such as having significant behavioral health needs (including qualifying for Specialty Mental Health Services or Drug Medi-Cal services), or having one or more serious <\/span><a href=\"https:\/\/mypacifichealth.com\/te\/what-is-the-connection-between-chronic-disease-and-mental-health\/\"><span style=\"font-weight: 400;\">chronic physical health conditions,<\/span><\/a><span style=\"font-weight: 400;\"> physical disabilities, intellectual disabilities, or developmental disabilities.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Be experiencing or at risk of homelessness<\/b><span style=\"font-weight: 400;\"> \u2014 as defined by <\/span><a href=\"https:\/\/www.hudexchange.info\/homelessness-assistance\/coc-esg-virtual-binders\/coc-esg-homeless-eligibility\/four-categories\/\"><span style=\"font-weight: 400;\">HUD guidelines,<\/span><\/a><span style=\"font-weight: 400;\"> with some <\/span><a href=\"https:\/\/policy-manual.mes.dhcs.ca.gov\/behavioral-health-services-act-county-policy-manual\/V1.0.0\/7-bhsa-components-and-requirements\"><span style=\"font-weight: 400;\">California-specific<\/span><\/a><span style=\"font-weight: 400;\"> modifications.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Be part of a transitioning population<\/b><span style=\"font-weight: 400;\"> \u2014 including individuals exiting a nursing home or long-term care facility, incarceration or carceral settings, the child welfare or foster care system, or interim housing.<\/span><\/li>\n<\/ol>\n<p><span style=\"font-weight: 400;\">This means Transitional Rent can serve a wide range of community members \u2014 from youth aging out of foster care to adults leaving institutional care to individuals managing serious mental health or <\/span><a href=\"https:\/\/mypacifichealth.com\/te\/national-prevention-week-substance-use-prevention\/\"><span style=\"font-weight: 400;\">substance use <\/span><\/a><span style=\"font-weight: 400;\">conditions. It&#8217;s designed to connect seamlessly with a broader network of services and supports through the CalAIM framework.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">The Housing Trio<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">When a member is approved for Transitional Rent, they are also automatically connected to <\/span><a href=\"https:\/\/www.chcf.org\/resource\/calaim-housing-community-supports-policy-at-a-glance\/#:~:text=The%20CalAIM%20Housing%20Trio%20is%20a%20set,in%20maintaining%20stable%20tenancy%20after%20moving%20in\"><span style=\"font-weight: 400;\">Medi-Cal&#8217;s Housing Trio<\/span><\/a><span style=\"font-weight: 400;\"> \u2014 three Community Supports designed to help members find, secure, and maintain housing:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Housing Transition and Navigation Services \u2014 assists with finding and applying for housing<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Housing Deposits \u2014 helps cover security deposits, first and last month&#8217;s rent, and utility setup fees<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Housing Tenancy and Sustaining Services (HTSS) \u2014 supports members in maintaining safe, stable tenancy after housing is secured<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">These supports can be received simultaneously, creating a comprehensive pathway from homelessness to long-term housing stability.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">Enhanced Care Management (ECM)<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Every member approved for Transitional Rent must also be authorized for <\/span><a href=\"https:\/\/mypacifichealth.com\/te\/enhanced-care-management\/\"><span style=\"font-weight: 400;\">\u0c2e\u0c46\u0c30\u0c41\u0c17\u0c48\u0c28 \u0c38\u0c02\u0c30\u0c15\u0c4d\u0c37\u0c23 \u0c28\u0c3f\u0c30\u0c4d\u0c35\u0c39\u0c23 <\/span><\/a><span style=\"font-weight: 400;\">(ECM). ECM providers assign a dedicated care manager to each member \u2014 connecting them to health care, behavioral health, medication management, education, and social services all in one coordinated plan. A <\/span><a href=\"https:\/\/mypacifichealth.com\/te\/community-health-worker\/\"><span style=\"font-weight: 400;\">community health worker<\/span><\/a><span style=\"font-weight: 400;\"> may also be assigned to help navigate community resources and ensure nothing falls through the cracks.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">ECM is especially critical for members with complex needs \u2014 those managing mental health conditions, coming out of a nursing home, or re-entering the community after incarceration. It ensures that housing stability is paired with the clinical and social support needed to maintain it.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">The connection between housing and health outcomes<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Stable housing is not a luxury \u2014 it is a health care necessity. Research consistently shows that homelessness drives higher rates of emergency department visits, worsens chronic disease management, and increases mortality risk.
