Providing Shelter & Safety through
the Medi-Cal Incentive

Providing safety measures for your safe keeping through housing and healthy meals to aid in the recovery from illness, living healthier lives, and avoiding higher, costlier levels of care.

Housing Transition Navigation Services

Focuses on providing support and services to individuals experiencing homelessness or at risk of homelessness to help them navigate transitions in housing and access appropriate healthcare services. The goal is to address the social determinants of health, including stable housing, to improve overall health outcomes for Medi-Cal beneficiaries.

Housing Tenancy and Sustaining Services

Focuses on providing support and services to Medi-Cal beneficiaries who are experiencing homelessness or at risk of homelessness. The primary objective is to help individuals secure and maintain stable housing while addressing their healthcare needs. HTSS is designed to improve health outcomes by addressing social determinants such as housing instability.

Short-Term Post-Hospitalization Housing

Focuses on providing temporary housing support to individuals who have been discharged from hospitals and are experiencing homelessness or housing instability. The goal is to ensure a stable and supportive environment for individuals recovering from hospitalization, addressing both their housing and healthcare needs. STPHH is part of the broader efforts to address social determinants of health within the CalAIM framework.

Personal Care and Homemaker Services

Focuses on providing in-home supportive services to Medi-Cal beneficiaries who need assistance with activities of daily living. This includes personal care tasks and homemaker services to support individuals in their homes, promoting independence and well-being. The aim of PCHS is to enhance the quality of life for beneficiaries by addressing their personal care needs within the CalAIM framework.

Housing Deposits

Focuses on providing support for housing-related needs by assisting eligible individuals with securing deposits required for housing. This is part of the broader effort to address social determinants of health, specifically housing stability, to improve overall health outcomes for Medi-Cal beneficiaries within the CalAIM framework.

Day Habilitation Programs

Focuses and aims to enhance participants’ skills, independence, and community integration through various activities and therapies. Day Habilitation may include educational, social, and recreational components tailored to the specific needs and goals of participants to enhance social and health determents needs.

Community Transition Services/ Nursing Facility Transition to a Home

Focuses on offering services that aim to facilitate the successful integration of individuals into community living arrangements after a period of care in a nursing facility, such as; Coordination, Home Modifications, Care Planning, Community Resources and much more.

Nursing Facility Transition/Diversion to Assisted Living Facilities

Focuses on offering caregivers a break from their caregiving responsibilities, allowing them time to rest, recharge, and attend to their own needs, such as; temporary care, in-home support, facility-based respite, flexible scheduling and much more.

Caregiver Respite Services

Focuses on offering a safe and stable environment for individuals who are no longer in need of acute hospital care but may require additional support before returning to their homes or communities. Recuperative Care aims to facilitate recovery, address social determinants of health, and prevent unnecessary hospital readmissions.

Environmental Accessibility Adaptations

Focuses on adaptations aim to remove barriers and improve the overall accessibility of spaces, allowing individuals with disabilities to navigate and participate more independently in various activities, such as; ramps and lifts, widened doorways, handrails and grab Bars, and much more.
