How Medi-Cal Members Are Getting Medically Tailored Meals — at No Cost

What are medically tailored meals, and how do they work?
Medically tailored meals are fully prepared meals designed around your specific health condition — created with input from a Registered Dietitian Nutritionist (RDN) and grounded in evidence-based nutrition guidelines. Whether you’re managing diabetes, heart disease, cancer, HIV, chronic lung disease, or a high-risk pregnancy, the meals you receive are tailored to support your body’s unique needs.
This is the “food as medicine” approach in action. The idea is simple: nutrition is not separate from health care — it is health care. When a patient receives the right nutrition at the right time, it can reduce the risk of hospitalization, help manage chronic disease, support recovery from injury, and dramatically improve quality of life.
As part of California’s CalAIM initiative, medically tailored meals fall under the broader umbrella of Community Supports — a set of 14 flexible services designed to fill gaps that traditional medical care sometimes misses. These gaps are often tied to social determinants of health: everyday conditions such as food access, housing, transportation, and language barriers that shape our overall health.
Who qualifies for this free benefit?
Eligibility is based on having a nutrition-sensitive health condition. Common qualifying conditions include:
- Diabetes (including gestational diabetes)
- Cardiovascular disorders and congestive heart failure
- Cancer
- HIV
- Chronic lung disorders
- Stroke
- High-risk perinatal conditions and pregnancy complications
- Chronic or disabling 정신 건강 conditions
Members with extensive care coordination needs may also qualify. Importantly, this benefit is not a food pantry or food insecurity program — it’s a clinical service. Meals must be medically necessary and tied to a specific health condition. DHCS (the California Department of Health Care Services) has confirmed that qualifying members are not excluded just because they may also be experiencing food insecurity — the two needs can coexist.

What does the meal benefit actually look like?
Once approved, members can receive up to two meals or meal packages per day, covering roughly two-thirds of their daily food intake. The benefit can be authorized for up to 12 weeks and may be reauthorized if it remains medically necessary.
Meals are delivered to your home — fully prepared and packaged for easy refrigerator storage. You don’t have to cook, shop, or figure out what’s “safe” to eat with your condition. That’s already been handled by nutrition experts.
This matters enormously for members who may be managing activities of daily living on their own, living in a residential care or assisted living setting, or relying on a caregiver for day-to-day support. When proper nutrition is delivered directly, it removes one major barrier to recovery and wellness — and gives both members and caregivers one less thing to worry about.

If you’re a member with complex health needs, you may also be eligible for 향상된 케어 관리. ECM is a high-touch care coordination benefit that connects you with a dedicated care team to help manage your health across all settings. Your ECM team can help identify your need for medically tailored meals, submit your referral form, connect you to a qualified provider, and make sure all your services work together seamlessly.
Other CalAIM Community Supports services that may complement your meal benefit include:
- Respite care — temporary relief for caregivers
- Habilitation services — support for developing and maintaining daily living skills
- Transitional support — help moving home from a facility like a nursing home or hospital
- Asthma remediation — home modifications to reduce health triggers
Together, these services reflect a commitment to treating the whole person — not just the diagnosis.
