How partnership works

One plan, one care team, every benefit.

Members keep the Medi-Cal plan they already have. Pacific Health Group joins their plan's care network as a contracted provider of Enhanced Care Management (ECM), Community Supports, and Behavioral Health services — at no cost to the member.

Health Plan of San Joaquin / Mountain Valley Health Plan

HPSJ / MVHP

A community-accountable, public Medi-Cal plan founded in 1996 to serve the Central Valley. In 2024 it expanded as Mountain Valley Health Plan to cover the Sierra and Mother Lode counties, becoming one of California's largest local Medi-Cal plans.
