Recovery doesn't happen alone. Our CFS team walks alongside Full Service Partnership clients and their families, caregivers, and chosen family — helping everyone navigate the behavioral health system, build skills, and stay connected to the supports that make recovery last.

How it works

Strengthening the families and communities that surround recovery — across Sonoma County.

Behavioral Health System Navigation

We help clients and families navigate complex systems of care, including Specialty Mental Health Services (SMHS), the Drug Medi-Cal Organized Delivery System (DMC-ODS), and the processes for accessing other local resources.

Patients' Rights Advocacy Support

Help understanding your rights within the behavioral health system and connecting to Patients' Rights Advocacy when you need someone in your corner.

Peer Support & Lived Experience

CFS staff are non-clinical mentors who draw on their own lived experience — so families feel understood, not judged, and see resilience modeled firsthand.

FSP Care Coordination

We participate in the client's Full Service Partnership care coordination so the voices of family, caregivers, and chosen family are part of the plan.

Educational Workshops

Regular workshops held throughout Sonoma County on skill building, psychoeducation, and how to access and use services — open to the public and designed to reduce stigma.

Resource Linkage & Clinics

Resource clinics and hands-on help accessing behavioral health, housing, health care, social services, and other community-based supports.
