An evidence-based psychosocial rehabilitation model where members and staff work side by side to rebuild confidence, friendships, employment, education, and housing — together.

Lifeline Phone
The federal Lifeline program provides a free smartphone, monthly minutes, unlimited texts, and high-speed data to eligible members. Pacific Health helps you apply, activate your device, and stay connected to your care team.
